    Report IPS office:

    Report in Police


    First of all, report the theft or loss of your passport in your nearest police station. You will need crime reference details to notify a loss or theft to Identity and Passport Service. To locate the nearest police station visit Metropolitan Police website.

    Get Lost or Stolen Passport Form


    After reporting to police, you will need to notify the loss or theft of your passport to the Identity and Passport Service in London. To report, fill in and sign a lost or stolen (LS01) notification form and return it to the IPS through post or in person. To get LS01 form please visit below given website:

    Filling Lost or Stolen Passport form


    There will be seven sections in LS01 form.


    1) In section 1 you will have to complete your personal details as follows.


    a)      Title:


    b)      Surname on Passport:


    c)       Forename on Passport:


    d)      Date of Birth:


    e)      Town of Birth:


    f)        Country of Birth:


    g)      Passport Number:


    h)      Was the passport issued in the UK:


    i)        Date of Issue


    2) In section 2 you will have to write current personal details but only if these are different than above.


    3) In this section you will have to write the details where and when your passport lost as follows:


    a)      Date of Loss:


    b)      Place of Loss (e.g. name of town, hotel, or airport)


    c)       Country of Loss


    d)      Postcode of loss


    e)      Circumstances of loss


    4) If you have reported a theft or loss of your passport, you will need to provide reference of your police report in this section. Following details will need to be enter:


    a)      Date of Report


    b)      Police Station


    c)       Police Report number


    5) You will need to fill this section only if you are not the passport holder and filing this form for someone else. You will give your full name.


    6) Now, if you are reporting for someone else, you will provide your full contact details.


    7) This section is your declaration that you have understood and read the full application form. You will write your name and sign it.
