Apology Email to Boss
You can offer your apology to the management or your boss directly through email. If you have failed to complete your tasks on time and have let the boss down on an occasion, the best way is to send an email to apologize for your actions. The first paragraph should include the full apology and you should explain what tasks you have failed to perform.
In the second paragraph, explain the reasons for your behaviour and try to defend your case, if there is any. In the last paragraph, assure the boss that this will not happen again and that you will show your full commitment towards work.

Respected Sir,
I am extremely apologetic for not complying with the company orders on visiting the site on Saturday. All the employees were advised to be there at 9 a.m. but I was unable to visit the site due to personal reasons. I know I should have informed the concerned personnel on my absence from office but the situation did not allow me to.
I want to let you know that my mother has been suffering from lung cancer and her situation suddenly worsened on Saturday and I had to rush to the hospital. I did not get the time to inform the office as the doctors said that her position is critical. However, by the grace of God, things are alright now and she is fine.
I certainly assure you that anything like this will not happen again and even if it happens, I will make sure that the department knows regarding my absence.
I, hereby, request you to forgive me for my actions and I will make sure this does not happen again.
