How to Fill Out a Self-Appraisal Form
As time changes, so do the ways in which things are done. In the past, employees were judged by their hard nosed bosses who were often unforgiving and extremely strict. The employee did not have a chance to defend him or her and had to please the boss at all times.
Now this has changed and better corporate practices allow people to be able to better defend themselves. Bosses and appraisals have also evolved. You can now do a self-appraisal in which you can judge yourself. It is something that has worked well for both the organisations and employees.

1
Be Honest
It is easy to make yourself sound like the right man for the CEO post when you are an internee but such an appraisal will not get you too far. You may have great skills but they can only be polished with time and you will improve as you become more mature. Be honest in the assessment of your skills and try not to be over smart when filling in the form. -
2
Know Your Strengths
This is very important. You must be aware of the strengths that you possess especially the ones that are needed by your current organisation. Highlighting them will help you in getting a positive response. It will also play an important role in the appraisal that will be done by the bosses and a reminder to them about your particular skills is never a bad idea. -
3
Know Your Weaknesses
This is also a must, partly because of the fact that your bosses certainly know them very well. Do not be shy on making a mention of areas that you feel you can improve upon. Be tactful and do not hand yourself all seasoned on a plate with a steak knife. Running yourself into the ground on the matter of weakness will not work in your favour. -

Achievements
You will also have a chance to list your achievements. If you are a good worker, you should have some in the period for which your appraisal is taking place. Make sure that you make a clear mention of them so that the organisation also knows that you are an asset. It is important that they know and you should remind them of the fact.
