    - Form 3949-A: If you have spotted false exemptions or deductions , kickbacks, failure to pay tax, failure to withhold, organized tax evasion or crime, unreported income in financial statements, and false documents.

    - Form 14039: If you suspect somebody has stolen your ID and is using your social security number for job.

    - Form 14157: If you spot a fraudulent tax scheme being run by a tax practitioner—individual or a company.

    Download the form you have selected. Print a hard copy and fill it out.
