Illness Leave Letter
Illness is never planned but unfortunately renders you unable to do much about it. If you recently fell ill and are unable to attend work, you will need to write an illness leave letter addressed to your supervisor or the human resources department. Here we will not only show you how to write an illness leave letter but will also give you samples and a template to follow when you are writing your own letter.
Tips for Writing an Illness Leave Letter
- Give some information about the illness you are suffering from.
- Quote your doctor if necessary.
- Give the exact dates you will be taking leave on.
- Give an expected date of return if possible.
- If necessary, you can leave your contact information for emergency cases.
- Offer to attach medical records and prescriptions if required.

Dear Alasdair,
I am writing this letter to request an illness leave for medical reasons. I have been suffering from Dengue fever since last night and the doctor has suggested one-week complete bed rest.
Given all this, I will not be able to attend office from October 19th, 2009 to October 26th, 2009. I can provide you with doctor’s prescription and medical reports, if necessary, and will try my best to make up for the lost time once I return to work.
Thank you so very much for your kind consideration.
Yours truly,

Dear Sir,
I am writing this letter to request a 3-day leave on account of illness. There is severe inflammation in my tonsils and I am unable to speak or eat without pain. Therefore, the doctor has advised an operation to remove the tonsils, and I will need to take time off from work for it.
I would like to take a week long leave, from November 23th, 2010 to November 29th, 2010. I know there is a lot of work which I need to do, but I will try my best to make up for it when I return.
Looking ahead for your kind response.
Yours sincerely,
